Cultural Issues and Role Playing

As oral health care professionals, we seek to improve the quality of life of all the clients in our care.

Role-play can help to begin the process. In order to feel the pulse of the generations before us, we have to make an effort to reach them by walking in their shoes.

Not only do we have to address the issue of age, but we have to be sensitive to the various cultures that the individual senior subscribes to. According to researcher Christopher Murray, the risk factors accounting for chronic disease are very strongly influenced by one’s culture and the social environment.

Understanding the cultural traditions of the elderly can enhance their quality of life. Communicating with seniors requires a general health assessment as well as thorough oral assessments. It is important to consider all of the predisposing, enabling and reinforcing factors present.

What kind of self care is the client able to perform? Will they need any assistance in carrying out the procedures recommended to maintain their oral health? What will keep them motivated to continue on the path of healthy living after their oral hygiene appointment? Klara Shekhtman of the Long Life Adult Day Care Center in New Jersey states that it is important to understand that if seniors don’t feel understood that it affects their general health and can lead to increased stress and depression. Effective communication throughout the treatment process is essential for maintaining health and well-being.

Personalizing Oral Health Instruction for the Geriatric Client:

Sample Student Assignment

Plan personalized self care instruction for one of the assigned clients:

● Research the specific needs and issues of your patient.
● Write a lesson plan including the anticipatory set, behavioral objectives, instruction, guided practice and closure steps.
Using role play, demonstrate the appropriate oral health aid for the specific needs of the client:
● The oral health instruction/demonstration must be delivered within a 5-7 minute time frame.

Sample clients:

● An 80 year old female with exposed root surfaces and medication induced xerostomia
● A 65 year old female with a history of periodontal surgery and root surface sensitivity
● A 70 year-old male with interproximal food traps and dexterity challenges due to stroke
● A 75 year old male or female with a 3 unit bridge from tooth numbers 29-31

Walking in Someone Else’s Shoes

One of the advantages of role-playing the elderly client is that in order to grasp the significant issues affecting this age group, the student has to research and reflect on the predisposing, enabling and reinforcing factors influencing behavior in this age group.

Assigning a senior/elderly character for role playing, provides students with opportunities to stretch their imaginations into the older client's life experiences. Students must also consider other intrinsic factors that play a part in the behavioral modification of the senior learner, including attitudes, values and motivation. Role play scenarios allow students the opportunity to momentarily walk in someone else's shoes and develop empathy for the multitude of issues facing seniors and the elderly.

As educators, we are responsible for encouraging the dental hygiene student to explore a variety of ways to communicate with the patient/client. Using role-play enables students to expand their critical thinking skills and provides an environment where they are able to discuss and analyze simulated situations distinctive to the elderly with the goal of increasing compassionate and effective oral care. The unique and complex needs of the elderly play a significant role in oral health care today and long into the future. It is imperative that we find ways to embrace this emergent population and provide students with the communication skills needed to deliver optimal care.

구강보건관리 전문가는 모든 환자의 삶의 질을 향상시키도록 해야 한다. 역할극은 이런 과정을 돕는다. 우리앞 세대를 이해하기 위해서 그들의 입장이 되어봐야 할 것이다. 나이에서부터 문화적인 것에 이르기까지 넓은 이해가 필요할 것이다. Christopher Murray는 만성질환을 일으키는 위험 요소는 문화와 사회적 환경에 의해서도 영향을 받을 수 있다고 했다. 노인의 전통문화를 이해하는 것은 그들의 삶의 질을 증진시킬 수 있다. 노인과의 의사소통은 일반적인 건강상태에서부터 구강건강까지 이루어질 수 있다. 모든 처치과정에서의 효과적인 의사소통은 건강과 안녕을 증진시키는 것이 핵심이라고 할 수 있다.

다른 사람의 입장에서 생각해 보기

노인 환자와의 역할극 가장 장점 중의 하나는 이세대의 영향을 주는 중요한 이슈를 이해하는 것이고 학생들은 이러한 연령대의 행동에 영향을 미치는 요소와 노인들의 경향성들을 연구하게 하고 강화하게 하는 것이다.

역할극에서 노인환자의 사정은 학생들이 노인들의 삶의 경험들에 대한 상상력을 확장시키는 경험을 제공하는 것이고, 학생들은 노인들의 행동수정을 일으키는 동기나 가치 등에 대해서도 고려한다는 점이다.

역할극 시나리오는 학생들이 다른 사람의 입장에 서볼 수 있는 기회를 제공하고 노인들이 직면하고 있는 상황에 대해서 감정이입 할 수 있는 능력을 증가시킨다. 교육자로서 교수들은 치위생과 학생들이 환자들과 소통할 수 있는 다양한 방법을 찾도록 격려해야 할 책임이 있다. 역할극을 이용함으로써 비판력을 향상시키도록 하고, 학생들에게 효과적인 구강증진행위를 증진시키기 위해 모의상황을 제공하고 이에 대해서  토론하고 분석할 수 있도록 해야 한다. 학생들이 최적의 구강관리를 제공하기 위한 의사소통 기술 방법을 찾는다는 것은 매우 중요한 일이라고 할 수 있다.
