치과위생사(미국)이면서 RDH magazine의 편집장인 Cynthia Mckane-wagester는 2002년도에 출판한 저서 Dental Hygiene : the Pulse of the Practice에서 단 한번의 치과위생약속(Hygiene appt.)을 통하여 유능한 치과위생사가 성공적으로 환자(고객)의 요구사항을 해결해 주고 구강건강 향상을 위한 인식변화 및 참여를 이끌어낼 수 있음을 하나의 시나리오를 통하여 소개하고 있다. 본 코너에서는 이 시나리오 안에서 이루어지는 치과위생사와 환자(고객) 및 치과의사와의 대화와 그에 대한 설명을 해석하여 원본내용과 함께 치과영어 회화 형식으로 회원들에게 소개하고자 한다.
Holly: Yes. See how the screen is divided into four sections. Well, we're going to have a different picture in each one. (Holly clicks and the lower right quadrant is fixed on the screen). When we're done, we'll print two sets. One for our file and one for you to take home. Now I'm going to get a picture of the lower left area that is giving you so much trouble. (Holly clicks again). Now let's move to the upper right. See the swelling in the gums here? Not too bad, but it needs some attention. I also notice that you have some fillings in this area. Most of them look very good. See how well they fit the contours of your teeth? But I want you to look at something over here. (Holly points to the far left of the screen). See this black area? It's a very large, leaking silver filling that is probably going to fall apart completely fairly soon. You will probably need a more secure restoration here. Let me get a picture of this so we can show Dr. Gentletouch, OK? (Holly briefly removes mouthpiece so that Mrs. Jones can reply).
Holly(치과위생사) : 그럼요. 여기 스크린이 4부분으로 나누어지는 것이 보이시죠? 자, 이제 우리는 각 부분에 다른 사진을 볼 수 있을 것입니다. Mrs. Jones: OK. Yeah, I can really see the difference between those two and that black one. That looks pretty bad.
Mrs. Jones(고객님) : 정말, 저 두개의 수복물과 하나의 검은색 수복물의 차이를 확실하게 볼 수 있네요. 검은색 수복물의 상태는 매우 좋지 않네요.
Holly: The filling isn't holding up too well. What would be best for the tooth would be to place a much more secure restoration, like a crown or an only. We'll discuss this with Dr. Gentletouch.

Mrs. Jones: Well, I've heard of crowns, but what's an onlay? And aren't they both expensive?
Mrs. Jones(고객님) : Crown은 들어본 적이 있는데 Onlay란 무엇인가요? 그리고 둘 다 비싸지 않나요?
Holly: An onlay is sort of a middle step between a crown and a filling. It keeps the tooth intact. If there is enough of the original natural tooth that's intact, then we use the onlay. the onlay or crown investment is approximately $850. Most insurance companies will cover approximately 40% of this cost, but even if you have to cover a portion of this cost yourself, it's a very good investment. We are very proud of the work we do here, and the crowns and onlays we place are very strong, solid, and permanent. We've performed this procedure on many other patients and they have been delighted with the results. Mostly, it's a way to prevent more serious problems. A good restoration now can save you a lot of problems down the road. That's pretty much our philosophy here, by the way. We're committed to preventive dentistry because we know it's a better investment than corrective dentistry. Also much cheaper.
Holly(치과위생사) : Onlay란 Crown과 수복물 사이의 중간단계로 분류할 수 있습니다. Onlay는 치아를 본래대로 유지시켜 줍니다. 완전한 상태의 자연치아가 충분하다면 Onlay를 할 것입니다. Onlay와 Crown의 가격은 약 850불입니다. 대부분의 보험회사에서 약 40%를 지불할 것입니다. 설령 고객님이 전액을 지불해야 한다고 할지라도 Onlay는 매우 효율적인 투자입니다.
저희 치과에서는 자부심을 가지고 매우 강하고 단단하고 영구적인 Crown과 Onlay를 제공하고 있습니다. 저희들은 많은 고객분들에게 이 과정을 실행해오고 있으며 고객분들이 결과에 매우 만족 해 하고 있습니다. 대개는 Onlay 또는 Crown가 더 심각한 문제를 예방할 수는 하나의 방법입니다. 좋은 수복물은 고객님께 앞으로 발생할 수 있는 많은 문제들을 줄일 수 있을 것입니다. 여담이지만, 그것이 저희 치과의 철학이기도 합니다. 저희치과에서는 치료중심보다 예방중심의 치과운영이 더 좋은 투자라고 생각하기 때문에 예방치과중심으로 전념하고 있습니다. 또한 훨씬 경제적이기도 하구요.
